DTECH Computerists (dTech) have been leaders in providing HRST -- health screening tools to public and private agencies serving the needs of persons with Developmental Disabilities (DD). This role has been expanded to encompass any given population of individuals with or without complex health care challenges. The dTech Group (DTECH Computerists) designed the Health Risk Screening/Assessment Tool (HRST-PSR) software in 1998 to increase the functionality of the "paper" version of this tool. The HRST-PSR (originally known as the Physical Status Review, or PSR) was developed in and by the State of Oklahoma and was originally designed to identify and track persons with potential life threatening health risks, who were moving to less restrictive community settings.
The HRST-PSR resides entirely on your computer/computers/workstations/servers. You can control your HRST database: it is local and you determine who will share in the information and who will not. We also provide the functionality whereby DTECH can also act as your HRST server and/or be a centralized backup repository. The HRST-PSR software is very mature and any bugs have been worked out years ago. The HRST reports are very sophisticated, as well as, being very thorough. We value our clients' input and continually upgrade the functionality of the HRST software based on outside suggestions. Our HRST add-on packages, such as Acuity-Based Staffing and Standards and Audits (or Standards and Guidelines), exemplify our openness to new functionality. We have gone from version 1.0 to the present version 6.1.
The HRST-PSR is the first in a series of software applications designed to increase the skills and productivity of direct care workers serving persons with disabilities due to age, accident or illness. Coming soon will be add-on modules dealing with behavioral problems and available now is an HRST acuity-based staffing add-on. The HRST staffing module will allow an administrator to determine the staffing levels, based on the HRST-PSR scoring, with the click of a mouse, as well as, perform budgetary estimations via acuity-based staffing projections. This HRST package was been in development for four years and has been fine-tuned to a high degree of accuracy.
The HRST-PSR was developed to screen for health risks associated with disabilities such as those associated with aging, physical injury, developmental insult, or other conditions affecting an individuals ability to engage in functional activities. Part of the HRST instrument examines the health risk associated with psychiatric or behavioral disorders, such as those resulting from the side effects of drugs, self-injurious behavior or restriction of movement.
The most important outcome of the HRST-PSR screening process is to guide the intensity and direction of health care support and surveillance. Many persons with physical disabilities are in relatively good health in spite of their apparent physical limitations. Most persons with developmental disabilities, for instance, enjoy health patterns equivalent to the population at large. Providing more health care than the persons health status indicates is a waste of resources that can be used in a more cost-beneficial way.
The HRST instrument has been available for more than eight years in a computerized form that allows clinicians and managers to identify those persons who need careful surveillance to prevent morbidity and mortality. The HRST-PSR can easily produce either individual HRST reports, or reports about an entire population, that assists in determining the nature and needs of the individuals served. Part of the HRST-PSR instrument examines the health risk associated with psychiatric or behavioral disorders, such as those resulting from the side effects of drugs, self-injurious behavior, or restriction of movement.
Over the last few years, modifications and additions have been made to satisfy the growing needs of the users and to better serve the community. The HRST-PSR has more reports, tracking of individuals between public and private institutions, mortality tracking, HIPAA Compliancy, Acuity Based Staffing projections, and Standards and Audits, to name a few.
The HRST-PSR is designed for field managers in who cannot adequately assess client health due to shortages of qualified workers and cost constraints. For these managers, The HRST-PSR is an information system that provides dynamic, electronic health monitoring of clients. Unlike existing, in-house, static paper systems, the HRST-PSR provides an easily implemented, affordable, standardized way for managers to track, report, and act on health trends through time and across service delivery settings.
HRST-PSR has a full set of reports: first, second, third tier, exceptional, and enhanced reporting. Enhanced HRST reporting capabilities include statistical analyses and comparisons between users' own records and those from other private and public facilities. These HRST reports and records will be considered proprietary and anonymity will be maintained between agencies. HRST Reports will be provided on a monthly and a quarterly basis, depending on function. Ad hoc HRST reports can be instigated by the user at a rate of up to two per month, 12 per year, and can be HRST reports that provide information on the misuse of the HRST-PSR function, use of multiple and new filters, etc. Storage and Enhanced reporting of DTECH stored clients' records will be $12.00 per year per client. . For 3000 or more clients, this will include two, two-day, user training classes for up to 30 students for each year this contract clause is in effect (normally $11,000). This fee provides not only the above capabilities, but also a built-in backup and disaster recovery function.
The HRST Acuity-Based Staffing (ABS) add-on allows for managers to predict staffing levels based on the HRST-PSR database: number of clients and their health risk levels. The HRST staffing module will allow an administrator to determine the staffing levels, based on the HRST-PSR scoring, with the click of a mouse. This HRST package has been in development for four years and has been fine-tuned to a high degree of accuracy. Also the HRST staffing add-on allows a manager to accurately predict staffing and budgetary salary costs for LPNs, RNs, Direct Care personnel, and Case Managers via the HRST-PSR database.
The HRST STANDARDS AND AUDITS (S&A) add-on package will round out the HRST-PSR. Depending on the scoring, the HRST report will print out sections that are relative to a consumer that specifies the HRST Standards, Guidelines, Procedures, and Audits that need to be, and should have been, followed. Actually the user has a choice of two HRST reports: a complete HRST report that spells out Standards, Guidelines, and Procedures, with the Audit format for each section, or the short HRST version with the only the Indicators and Audit format.

The Health Risk Screening Tool- HRST- analyzes health indicators in 22 areas, captures relevant case notes, produces risk scores that identify the need for nursing intervention, and generates a set of training and service recommendations based on the needs of the individual. The HRST risk score for a specific individual can be compared through time to identify health trends that left uncorrected could result in a loss of functional skills, acute health problems, costly hospitalizations, intensive nursing care, or death. Risk scores viewed across individuals identify agency patterns of care and needs for professional services and staff training.
The HRST-PSR analyzes health indicators in 5 rating categories. The HRST instrument assigns point scores to 22 rating items, with the resulting numerical totals determining HEALTH CARE LEVELS associated with DEGREES OF HEALTH RISK. Six HRST health care levels which are associated with LOW, MODERATE OR HIGH HEALTH RISK are the direct outcome of the process, which is completed by persons who are often non-clinicians, but who are trained to use the tool.
The HRST instrument captures relevant case notes, and the risk scores are used to identify the need for further assessment and intervention, such as medical, therapy, behavioral, and nursing. HRST Staff training and support recommendations are also provided.
HRST Risk scores for specific individuals can be compared through time to identify health trends that, left uncorrected, could result in an increase in acute health problems, costly hospitalizations, loss of functional skills, need for intensive nursing care, or death. HRST Risk scores across individuals identify agency patterns of service need, professional, and direct care staffing requirements.

Some anecdotal evidence of the power of the HRST-PSR is evidenced in the following "war stories".
1) One State that uses the HRST-PSR extensively used the information on "Emergency Room Visits" to justify hiring medical doctors to be on staff full time at the State Centers. This information is available in one of the many HRST reports provided by our reporting system. The emergency room visits were costing, on average, about $1500.00 each. What at first seemed like a good idea, was unjustified quickly six months later, when the same report was run and it was discovered that with the doctors on staff, the emergency room visits almost doubled. It was obviously a case of doctors not wanting to take responsibility and shunting consumers off to the hospital at the least provocation.
2) A private provider with 300 consumers in its care, and in a State that uses the HRST-PSR to determine consumer care costs, claims that they save hundreds of thousands of dollars each year on just the RN staff payroll costs by using the HRST-PSR. The HRST Acuity-Based Staffing (ABS) add-on package can determine how many hours are needed for each consumer, and the staff can be scheduled accordingly.
(HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT OF 1996 and Canada's Personal Information Protection and Electronic Documents Act)
The applicable HIPAA rules of concern to the HRST-PSR are:
1. In Title II, Administrative Simplification, HIPAA endeavors to minimize the amount of paperwork and human intervention; and
2. In Title V, The Standards for Privacy of Individually Identifiable Health Information are imposed to help guarantee privacy and confidentiality of patient medical records.
In the area of Title II, the HRST-PSR will definitely simplify and reduce paperwork via the HRST computer database and extensive HRST reporting capabilities.
In the area of Title V, 45 CFR Parts 160, 162, and 164, Health Insurance Reform: Security Standards; Final Rule, published 02/20/2003, the HRST-PSR is HIPAA compliant in that:
The HRST program is multi-level, password protected,
The HRST database is encrypted programmatically and uses database passwords,
The HRST files that can be output via floppy or electronic transmissions are encrypted,
And the HRST reports that contain consumer names or social security numbers can be designated to produce gibberish for those items. This allows a user to produce HRST reports that can be given to another agency, for instance, which will have all the HRST statistical data, but no discernible personal HRST data.
Canada's Personal Information Protection and Electronic Documents Act pertains to the same protections for personal data but also requires the collecting agency to:

Acuity-Based Staffing -- ABS
The Health Risk Screening Tool -- HRST Acuity - Based Staffing -- ABS -- add-on allows for administrators to predict staffing levels in nursing, direct care, and case management based on the HRST-PSR database: number of clients and their level of health risk (low, moderate, or high). The HRST staffing module will allow an administrator to determine the staffing levels with the click of a mouse. This HRST package has been in development for four years and has been fine-tuned to a high degree of accuracy.
The HRST staffing add-on allows a manager to accurately predict staffing and budgetary requirements for LPNs, RNs, Direct Care personnel, and Case Managers via the HRST-PSR database. HRST allows further refinement through a drop down whereby "ALL" sub-units may be selected, or just one sub-unit. Sub-units can be input, and can represent buildings or wards for a facility, or residence addresses for the Community. This HRST database contains the health risk levels that have been calculated and the hours per employee per client by HRST-PSR level and by employee type, that are needed to adequately care for a large, or small group, of clients. The hours needed were determined empirically over a period of two years and involved two state DD departments. This HRST model allows the input of different designated relief factors for each type of employee. The caseload and weighting can be based on acuity for Case Managers. The HRST staffing module will allow an administrator to determine the staffing levels, based on the HRST-PSR scoring, with the click of a mouse, as well as, perform budgetary estimations via HRST acuity-based staffing projections.
This HRST ABS module is a predictor of staffing needs. When used properly, a manager can see when the number of clients or health risk levels have risen or fallen, and can accurately determine the staffing levels needed at that precise moment in time. This is not only a great time and money saver, but it also allows for the best possible care to be provided, by the most appropriate staff.
In the standard HRST package, we do not allow the user to change the hours per client, or the Case Manager weighting. This is an effort to establish a standard. However, we can supply HRST versions with the input screens available, allowing the user to change the hours that have been empirically determined. Of course, in the standard HRST version the user can also make changes to the overall outcome by varying the Designated Relief Factors.

editing capabilities. HRST Server license at central location with HRST database
only. DTECH can provide redundant HRST client record storage2 and
enhanced HRST reporting capabilities1 and HRST Training6
for a charge of $12.00 per HRST client per year.
UNLIMITED
SINGLE USER LICENSES WITH DTECH STORAGE AND REPORTING - $36,000.00+ - With a five year
minimum contract: unlimited HRST users' licenses3 with electronic HRST-PSR
record exporting/uploading, full HRST reporting5, and full HRST editing
capabilities and HRST training6 will be provided. A minimum of 1000 unique
HRST-PSR client/social security numbers, or $36,000.00 per year is required, with an
additional charge of $12.00 per HRST client over 1000. This is basically the
SINGLE-TIERED, SINGLE-USER (ELECTRONIC) HRST version using DTECH as the HRST server.
UNLIMITED SINGLE USER LICENSES WITH DTECH STORAGE AND REPORTING - $240,000.00+ - With a five year minimum contract: unlimited HRST users' licenses3 with electronic HRST-PSR record exporting/uploading, full HRST reporting5, and full HRST editing capabilities and HRST training6 will be provided. A minimum of 10,000 unique HRST-PSR client/social security numbers, or $240,000.00 per year is required, with an additional charge of $24.00 per HRST client over 10,000. This is basically the SINGLE-TIERED, SINGLE-USER (ELECTRONIC) HRST version using DTECH as the server.
1 Enhanced HRST reporting capabilities include statistical analyses and comparisons between users' HRST own records and those from other private and public facilities. These HRST reports and HRST records will be considered proprietary and anonymity will be maintained between agencies. HRST Reports will be provided on a monthly and a quarterly basis, depending on HRST function. These HRST special reports can be instigated by the user at a rate of up to two per month, and can be HRST reports that provide information on the misuse of the HRST-PSR function, deaths, etc. Also, the user can request up to twelve new, different, unique, ad hoc, HRST reports per year. These HRST reports can be of a complexity that will amount to two or less programmer-days to accomplish. Once programmed, these HRST reports can also be requested on a frequency of monthly, quarterly, semi-annually, or annually.

DTECH will provide automatic dial-up or Internet connection software to facilitate the downloading of any HRST data. This HRST function will inherently provide offsite, backup storage for HRST client data. The client will have to provide a dedicated line, or TCP/IP address. The alternatives to dial-up are: email the HRST files to DTECH, or mail a floppy, all with safe, encrypted files. The creation of the HRST files is a daily, automatic process.3
An HRST user's license is defined as a single version of HRST-PSR running on a single HRST computer with a multiple HRST users.Limited Rights as to Use of the HRST Software
DTECH grants a non-exclusive, non-transferable HRST license to:
(a) Install and use the HRST Software on a single personal computer (not on a computer network) in machine-readable form for the sole purpose of accessing the services provided;
(b) Install the HRST Software on another personal computer if necessitated by a failure of the first personal computer, so long as the HRST Software is not used on more than a single personal computer; and
(c) Maintain one backup copy of the HRST Software for archival and recovery purposes.

Storage and reporting of DTECH-stored HRST consumer records will be $12.00 per year per HRST consumer -- based on quarterly HRST reports. Payment may be quarterly, semi-annually, or annually in advance. An HRST record for billing purposes is defined as a unique social security number. Many HRST-PSR records can exist for a given client/social security number, but each unique client/social security number will count as one. So it is very important to use only one social security number per client, even though some may have multiple social security numbers. This ensures proper HRST reporting and payments.5
Reporting capabilities will be defined as such:The first tier will be the reporting that pertains to an individual HRST user, as in the SINGLE-USER (#1 and #2 above) versions.
The second tier will be reporting that pertains to an individual HRST location, as in #3 through #6 above.
The third tier reports will be where DTECH provides HRST reports that pertain to multiple HRST locations, as in #7 through #10 above. This also means DTECH has the HRST consumers' data on the DTECH server.
HRST Exception reporting will be where DTECH provides HRST reports that pertain to multiple HRST locations for exceptional problems, such as invalid social security numbers, as in #7 through #10 above.
HRST Full Reporting will be defined by all three tiers of HRST reporting, plus the exception reporting, as in #7 through #10 above.
Enhanced HRST reporting capabilities include statistical analyses and comparisons between users' own HRST records and those from other private and public facilities. These HRST reports and HRST records will be considered proprietary and anonymity will be maintained between agencies. HRST Reports will be provided on a monthly and a quarterly basis, depending on function. These special HRST reports can be instigated by the user at a rate of up to two per month, and can be reports that provide information on the misuse of the HRST-PSR function, deaths, etc. Also, the HRST user can request up to twelve new, different, unique, ad hoc, HRST reports per year. These HRST reports can be of a complexity that will amount to two or less programmer-days to accomplish. Once programmed, these HRST reports can also be requested on a frequency of monthly, quarterly, semi-annually, or annually.
6 Training
DTECH will also provide two, two-day, HRST user training classes for up to 30 students for each year this contract clause is in effect (normally $15,000). This free HRST training requires a minimum of 3000 HRST consumers on the $12.00 per HRST consumer per year fee. This HRST training will prepare the HRST users to use the HRST instrument correctly: the HRST instrument can only be as effective as it is accurate in reflecting the health status and needs of individual consumers. Two instructors and a free HRST Rater's Guide manual for each student are normally provided for this HRST course.
HRST-PSR UPGRADES
Any multi-user, HRST Subscription version can be upgraded to an HRST network version and HRST standard subscription pricing for $2500.00.

There is no standard upgrade path for single-user, HRST versions. We will discuss all options though.

HRST ABS, the HRST Acuity-Based Staffing add-on, is priced at $10.00 per HRST client, minimum $1000.00, and a maximum of $10,000.00 per package.
